First Visit

Your initial appointment will consist of a comprehensive examination including an oral cancer screening and any necessary digital X-Rays. Upon completion of examination, a consultation explaining diagnosis and treatment options will be reviewed with you.

Please assist us by providing the following information at the time of your consultation:

• Please print and complete the new patient forms provided on our website. Submit online, e-mail or mail back forms one week prior to your visit.
• A list of medications you are presently taking.
• Photo ID & Insurance card

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
